How much has anxiety been affecting you lately?

A free, confidential 2-minute screening based on the GAD-7, the same 7-question tool clinicians use. Nothing you enter is stored or sent anywhere — your answers are scored right in your browser.

Over the last two weeks, how often have you been bothered by the following problems? Answer as honestly as you can — there are no right or wrong answers, and nothing you enter here is saved or sent anywhere.
1Feeling nervous, anxious, or on edge
2Not being able to stop or control worrying
3Worrying too much about different things
4Trouble relaxing
5Being so restless that it is hard to sit still
6Becoming easily annoyed or irritable
7Feeling afraid, as if something awful might happen
8If you checked off any problems, how difficult have they made it for you to do your work, take care of things at home, or get along with other people?
Answer all 7 questions to see your score.

This screening uses the GAD-7 (Generalized Anxiety Disorder 7-item scale), developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues. It is a screening tool, not a diagnosis. Only a licensed clinician can diagnose an anxiety disorder, and a high or low score here does not replace a professional evaluation. If you are in immediate danger or thinking about hurting yourself, call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.

What to do with your result

A screening score is a starting point, not a verdict. If your score was in the moderate or severe range — or if anxiety is getting in the way of work, sleep, or relationships regardless of the number — the next step is a conversation with a clinician. At Forrest Behavioral Health that starts with a short, confidential call: we listen, check your insurance for free, and tell you plainly which level of care fits, whether that is weekly therapy, our Intensive Outpatient Program, our Evening IOP built around work and family, or our Day Treatment Program.

Anxiety is one of the most treatable conditions we see. Have questions about how our programs work? Read our IOP & PHP FAQ.
