Has your mood been weighing on you lately?
A free, confidential 2-minute screening based on the PHQ-9, the same 9-question tool clinicians use to screen for depression. Nothing you enter is stored or sent anywhere — your answers are scored right in your browser.
Talking it through with someone takes about ten minutes, is confidential, and doesn't commit you to anything.
Call (781) 570-5781 Verify insurance Depression & mental health treatmentThis screening uses the PHQ-9 (Patient Health Questionnaire-9), developed by Drs. Robert L. Spitzer, Janet B.W. Williams, Kurt Kroenke and colleagues. It is a screening tool, not a diagnosis. Only a licensed clinician can diagnose depression, and a high or low score here does not replace a professional evaluation. If you are in immediate danger or thinking about hurting yourself, call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.
What to do with your result
A screening score is a starting point, not a verdict. If your score was in the moderate range or higher — or if low mood, exhaustion, or hopelessness is getting in the way of work, sleep, or relationships regardless of the number — the next step is a conversation with a clinician. At Forrest Behavioral Health that starts with a short, confidential call: we listen, check your insurance for free, and tell you plainly which level of care fits, whether that is weekly therapy, our Intensive Outpatient Program, our Evening IOP built around work and family, or our Day Treatment Program.
If you had any thoughts of hurting yourself, please don’t wait — call or text 988, or call us today. Depression is one of the most treatable conditions we see. Learn more about depression treatment or read our IOP & PHP FAQ.
